Research

The expectations of patients with hip and knee arthritis.
I have evaluated the expectations of 60 consecutive patients awaiting revision hip arthroplasty who were assessed with the SF-36, WOMAC, and expectation WOMAC questionnaires in order to determine the relationship between patients’ pre-operative symptoms and scores and their expectations. The post-operative outcome on these patients is being determined at present.
We are prospectively comparing the expectations, health profiles and neuropsychological scores of patients with SLE, Rheumatoid Arthritis and Osteoarthritis of the hip and knee. The data collection from this longitudinal study is now complete

The expectations of patients undergoing arthroscopic knee surgery and ACL surgery
We are evaluating the expectations, health profiles and activity levels of patients undergoing arthroscopic knee surgery and ACL reconstruction. This may help to predict the rehabilitation regimes required and to identify predictive factors for the success of such surgery and for return to pre-operative activity.

The use of Open MRI for the assessment of acute knee injuries
We have prospectively entered 50 patients with acute knee injuries into a study where a careful clinical assessment is made, an open MRI scan is obtained and the results of arthroscopic examination by the same surgeon are recorded. We are now collating the comparative data. Our findings will clarify the usefulness of open MRI examination of the knee for acute injuries, and may have important economic consequences

The use of Open MRI for the assessment of patellar tracking.
We have prospectively undertaken tracking MRI scans in 30 patients with acute and chronic patellofemoral tilt / subluxation / dislocation. The MRI findings will be compared to standard radiographic parameters and to surgical outcomes in order to clarify the importance of certain MRI observations such as buckling of the lateral retinaculum.

An EMG Study of quadriceps activity in patients with patellofemoral problems performing functional activities
The purpose of this study was to evaluate any differences in quadriceps activation during functional activity between patients with patellofemoral problems and normal subjects. Patients activated their VMO significantly less effectively than controls. In the controls, most of the activity was seen with the knee in flexion, with very little activity with the knee in extension. In comparison, patients recorded less activity with the knee in flexion and more when the knee approached extension. Our data also suggests that the motor control of VMO/VLO may be different from the bulk of the quadriceps. Our findings have implications for both the pathogenesis and the treatment of patellofemoral pain. This work is ongoing and preliminary data has been presented.

The use of gene physiology to assess the physiology of bone turnover
There is a strong genetic component to the determination of bone mineral density and mass, but the fundamental cellular and biochemical processes that regulate normal physiological bone deposition and resorption have yet to be elucidated. Out pilot data suggest that angiotensin I-converting enzyme (ACE) and interleukin-6 (IL-6) may have an important role to play. The aim of this study is to use a novel genetic strategy to investigate the mechanisms that regulate normal physiological bone turnover. We are examining the relationship between gene polymorphisms and skeletal muscle mass and response to training in a cohort of army recruits. Our preliminary results have wide reaching consequences with regards to sporting ability and resistance to repetitive stresses. This work may have potential implications in the management of osteoporosis, bony metastases and juvenile rheumatoid arthritis. . Data will be available for presentation and publication in early 2004.

We are also collaborating nationally and internationally to use gene polymorphisms to study bone physiology / turnover in myeloma, bone malignancies, stress fractures and bone loss after immobility / intensive care.

Cerebral micro-emboli during hip and knee arthroplasty surgery: a cause of for cognitive decline
There is very limited information regarding the incidence of cognitive decline following hip and knee replacement surgery. This study will specifically examine both neuropsychological and orthopaedic outcomes and their relationship to microemboli detected during hip and knee replacement surgery. In addition the relationship between the changes in cognition and quality of life will also be assessed.

Impaction grafting in revision hip surgery
We are undertaking laboratory study and animal studies to evaluate the impact of various bone substitutes and surgical factors on the mechanical properties of impaction grafting reconstructions.

Proximal Tibial Anatomy in relation to TKR
I have already published a paper on the effect of high tibial osteotomy on subsequent total replacement. This study can be linked to ongoing work on proximal tibial morphology in patients undergoing total knee replacement. I am undertaking a comparison of 100 North American knees and 100 UK knees. We are measuring the AP and lateral radiographs of these patients to determine the optimal AP and lateral offset at 0, 0.5, 1, 1.5, 2.0 and 2.5cm below the joint line to give the best tibial tray coverage with an intramedullary stem. This will help to guide the design of future revision prostheses, and has implications for surgical planning.

Novel Bearing Surface – Oxinium
I have set up a multicentre study comparing oxinium femoral heads with cobalt chrome and standard polyethylene with highly cross linked polyethylene in THR.
A number of laboratory studies(simulator studies, surface analysis, retrieval) with oxinium are also ongoing.

Periprosthetic Fractures around Hip and Knee Replacements
I have completed clinical and biomechanical studies on the fixation of periprosthetic fractures. I am undertaking further biomechanical work. I am also gathering data on the epidemiology and management of periprosthetic fractures in the UK to compare this to North American data that I have already collated. There appear to far fewer patients with massive bone loss at the time of periprosthetic fracture in the UK at present. This could have implications with regards to the follow-up of hip replacements….

There is on-going work and collaboration with Robert Barrack and Michael Ries in the United States on improving the cabling of fractures and trochanteric fixation. Some of this work was presented at the American Academy of Orthopaedic Surgeons meeting in San Francisco. There will be ongoing work in this area and a number of future projects and presentations.

Vitamin D in the treatment of osteoarthritis and its potential effect on the outcome of hip and knee replacements
We have collected baseline data on Vit D levels and PTH in patients undergoing THR and TKR.
We have started a randomised, double-blind, placebo-controlled trial of vitamin D supplementation in the management of symptomatic knee osteoarthritis. We are also assessing the effect of Vit D on periprosthetic changes after THR and TKR.

Prospective evaluation of hip and knee joint arthroplasty in patients with Paget’s disease
Previous studies suggest that Paget’s disease patients who have undergone hip arthroplasty have poorer implant survival rates when compared to patients with primary osteoarthritis undergoing similar surgical procedures. This study will prospectively evaluate the outcome for hip and knee replacements in Paget’s disease patients to determine if advances in medical management and surgical techniques have also resulted in improved long-term implant survival and symptom control. We are starting data collection with the support of the Paget’s association.

The role of bone scans in the evaluation of the painful hip replacement
We have evaluated the sensitivity and specificity of Tc99 Bone Scans in the assessment of hip pain by reviewing the long-term outcome of 100 arthroplasties investigated in this way. This has now been extended into a prospective study evaluating PET scans and white cell scans for a similar cohort of patients.

Outcome studies in lower limb reconstruction
I have spent the last four years setting up a variety of outcome modules and outcome studies after ACL reconstruction, total hip and total knee replacement as well as revision surgery at University College London Hospitals.

We have set up collaborations with our anaesthetic, ITU and neuropsychology colleagues. At present there are studies ongoing into risk assessment and risk scoring in joint arthroplasty patients, into expectations both in disease specific situations and in relation to surgical intervention for sports knee reconstructions as well as arthroplasties and revisions, into quality of life prior and after surgery and functional measures both in primary and revision joint replacement.

I have instituted a programme of data collection within integrated care pathways, data bases and studies that have allowed us to create an expanding database of joint replacement outcomes and we are in the process of evaluating psychometric data, inflammatory markers and functional tests in the early post-operative period to correlate with medium term and longer term outcomes.

We are on the verge of publishing our first papers on the use of the POMS scoring system looking at peri-operative morbidity and its impact on the economics of hospital admission and hospital stay.

We are now in a position to look at quality of life measures and functional measures in relation to primary and revision joint replacement and compare these.

I have treated over 100 patients with peri-prosthetic infections which have been very carefully recorded and data based and have been treated according to a standardised protocol which will be ready for publication within the next year.This has included a prospective randomised study of static versus mobile spacers for infected TKR.

There is on-going work and collaboration with Robert Barrack and Michael Ries in the United States on improving the cabling of fractures and trochanteric fixation. Some of this work was presented at the American Academy of Orthopaedic Surgeons meeting in San Francisco. There will be ongoing work in this area and a number of future projects and presentations.

General Interests
& Activities I am fluent in French and Arabic and have travelled widely.
I was a keen rugby player and played for United Hospitals and several other representative sides during my medical training. I am a Vice-President of University College and Middlesex Hospital’s Rugby Club. I am club Orthopaedic Surgeon to a number of Premiership football and Rugby clubs.
I still play tennis. I am also very interested in running and fitness training, and have worked in conjunction with personal trainers for several years.
I am also an avid bridge player.
